OFFICE VISITS
New Patient Consult Appointment — $385.00
New ponsult appointments can be face-to-face or via phone or Zoom for long-distance patients—the cost is the same.
Follow Up Appointment — $175.00
A follow up appointment is needed to review all lab, saliva test and thermal image results. The follow up appointment is NOT included in the cost of any lab work or testing.
TESTING
Hormonal and Adrenal Function Testing
There are a variety of testing levels available, from $210 for a Basic Hormone Panel to $420 for a Comprehensive Hormone Panel.
Other Testing
Other individualized health testing costs vary, depending on the severity of your condition and the necessity for testing. This will be discussed at your initial visit.
MEDICAL THERMAL IMAGING
Breast Screening — $215.00
Head and Neck Screening — $175.00
Single Extremity Screening — $125.00
Additional Views — $75.00 per view
Thermal Imaging Review — $175.00 or $200.00
Reviewing Thermal Imaging results and getting recommendations from Lynn regarding results requires a followup appointment that is not included in the cost of the Thermal Imaging itself. Call the office and make a follow up appointment for Thermal Imaging Review.
If you see Lynn for other healthcare services besides Thermal Imaging, Thermal Imaging Review is $175.00
If you see Lynn for Thermal Imaging only, Thermal Imaging Review is $200.00
Self-billing insurance (other than Medicare) may be possible. Please talk to your insurance company directly.
If you are interested in self-billing your insurance, please be aware of the following:
NOT all plans allow self-billing, and Medicare does NOT permit patients to self-bill.
It is the responsibility of the patient to consult their insurance plan for instructions and appropriate forms for self-billing.
At the time of your visit you will receive an itemized receipt with necessary insurance codes to facilitate self-billing.
It is advised to make copies of all forms prior to submitting insurance claims.
Lynn Chadd is an Out-of-Network Provider/Non-Preferred Provider who is not contracted with any insurance companies.
CANCELLATION
A 48-hour notice is required for all patient appointment cancellations.
We reserve the right to charge in full for your appointment if you:
cancel with less than 48-hour notice
fail to show up for your scheduled appointment time
